Join Dr. Peter Weir from the University of Utah as he discusses their Intensive Outpatient VBC Model for High Risk Medicaid Patients.
Objectives:
1. Understand how a value-based care model can sustainably support a clinic exclusively focused on Medicaid members with complex medical, social, and behavioral health problems.
2. Become familiar with a claims-based approach to identify high-risk Medicaid members that can be served in an interdisciplinary clinic.
3. Discuss how a trauma-informed model is foundational in caring for a cohort of patients that historically over utilize healthcare resources.
